The multi-disciplinary Homeless Outreach Stabilization Team (HOST) will work in the community to provide services which includes substance abuse treatment, medical services, rehabilitative support, case management and peer support services.

The purpose of this program is to serve homeless individuals with behavioral health challenges who have demonstrated a combination of continuous service needs and functional impairments who have not shown to benefit from traditional outpatient programs currently available.

The SUDP will work collaboratively with a multi-disciplinary team serving homeless adults experiencing severe and persistent mental illness and substance use disorders The SUDP is a member of a multi-disciplinary team in expertise with substance use disorders. SUDP will collaborate with medical team, provide relevant psychoeducation to the larger team, and work directly with clients on practicing safer use, including access to supplies. Outreach and engagement locations include emergency homeless shelters, transitional homeless shelters, homeless encampments, transportation hubs, train stations, and other pertinent community location where people may gather.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.
  • Perform alcohol and drug assessments, and intake evaluations on selected clients; with the client's participation, develop support and treatment plans; facilitate linkages to collaborative resources when appropriate.
  • Plan, organize and facilitate treatment and support groups for clients, as appropriate.
  • Provide staff/client training on substance abuse issues as appropriate.
  • Participate as a member of a multi-disciplinary team providing ongoing advocacy and integration of services to homeless individuals.
  • Advocate for client access to community resources and services, ensuring that client needs are met and rights maintained; consult and collaborate with community providers to ensure continuity of care.
  • Participate in psychiatric consultation, supervision, program meetings and in-service trainings; participate in clinical reviews and case conferences for clients on caseload.
  • Perform FBH charting requirements, correspondence, Workday and other paperwork in a timely manner.
  • Comply with the agency's clinical accountability policies and procedures; maintain current and complete clinical records; participate in quality assurance reviews when assigned.
  • Other duties as assigned.
QUALIFIED CANDIDATES WILL HAVE:

REQUIRED:
  • Associate's degree in Addiction Studies or Substance Use Counseling or a related degree type.
  • Must be credentialed as a Substance Use Disorder Professional.
  • Must apply for Agency Affiliated Counselor Registration within 30 days of hire.
PREFERRED:
  • Bachelor's or Master's degree in behavioral health or related field.
  • Two years' experience working with psychiatrically impaired populations.
KNOWLEDGE, SKILLS & ABILITIES:
  • Must have reliable transportation, insurance and an active driver's license and be willing to use personal vehicle in the course of work.
  • Ability to communicate and engage effectively with individuals of varied, backgrounds, trauma experiences, cultures, education level and socio-economic status.
PAY RANGE (Hourly/ Non-Exempt): Depends on degree level as noted below:
  • Associate's Degree - $25.53 - $31.10
  • Bachelor's Degree - $27.59 - $33.24
  • Master's Degree or higher - $33.53 - $40.48
All pay is based on longevity at Frontier Behavioral Health; thus, the starting pay for this position is as follows:
  • Associate's Degree - $25.53
  • Bachelor's Degree - $27.59 per hour.
  • Master's Degree or higher - $33.53 per hour.
