Responsibilities for Discharge Planner: Conduct rounds with physicians to meet with patients to discuss placement orders. Assist patients with a specific plan for medication management. Include patient in discharge planning. Assist patients with the identification of a support network, instructions for future care and follow-up. Coordinate care with community agencies, shelters, employment agencies, and drug and alcohol counselors Qualifications Qualifications for Discharge Planner: Bachelor's Degree in a human service field. Knowledge in the development of discharge plans and coordination with community service providers. Valid CPR card....Planner, Healthcare, Business Services, Patient
