The MSW Manager of Social Work Case Management provides daily leadership and oversight to a team of clinical professionals, ensuring the delivery of high-quality hospital-based social work services and seamless Care Coordination across the continuum. This role collaborates closely with the director, market leaders, physicians, community partners, and other stakeholders to identify and implement opportunities for enhancing patient care and services. Through strong partnerships and a commitment to best practices, the manager supports the system's mission of patient-centered care and active community involvement.

Qualifications:

  • MSW required
  • Medical Master Social Worker required
  • Leadership in Acute Hospital setting MSW leadership experience required

Responsibilities:

  • Ensures that the division mission, vision, and values are actualized, and that work is evaluated against those principles.
  • Ensure that patients experience seamless and safe transitions of care.
  • Ensures that patient and family engagement are an integral and essential component of the care delivery process.
  • Rounds frequently to observe patient engagement, physician relationships, and communication with other team members.
  • Oversees staff and provides direction and guidance, including clinical supervision where appropriate. Assesses clinical competency of Hospital Social Work staff.
  • Administers management functions within the provisions of policies and standards for federal, state, and local regulation.
  • Develops trusting relationships with assigned RN Care Managers, Social workers and physician providers; works closely with the department Social Work Manager to optimize teamwork in support of patient transitions.
  • Develops significant market savvy and ensures that services and interventions address the needs of the market demographic, and are sensitive to cultural and ethnic practices and beliefs.
  • Understands community health challenges and works with market leaders and division staff to develop and implement practices, sites of care, education programs, and other strategies to improve the health of the community and ensure continuous improvement in health status across the continuum.
  • Works quickly to address barriers and challenges associated with cross continuum care coordination and engages colleagues and staff in hardwiring appropriate solutions.
  • Ensures that staff have access to pertinent, ongoing, relevant education that contributes to professional knowledge and enhances practical abilities. Covers patient assignments if needed to ensure staff attend required education.
  • Establishes rapport and relationships with hospital and community physicians and physician practices, supporting physicians in navigating the complex challenges of compliance and cost-effective care.
  • Models and orchestrates consistent, professional communications at the tactical and strategic level.
  • Stays abreast of industry trends and regulations and serves as a resource for staff, colleagues, and physicians.

Compensation:
$137,906 per year
