Job Details

Provide housing navigation & case management services to participants co-enrolled in SSVF (Supportive Services for Veteran Families) & VA Medical Center, HCHV (Health Care for Homeless Veterans) Outreach Center. Work with referred veterans & their families to assess housing resources, needs, barriers & make an appropriate housing plan cooperatively with the Veteran & execute plans toward housing stability & permanence. Provide ongoing case management to participants with pending HUD (Housing & Urban Development) -VASH (Veterans Affairs Supportive Housing) housing vouchers. Position requires the ability to drive for company business which includes a valid driver's license, clean driving record, & proof of personal auto insurance. Must pass driver record & background check.n.
