We are seeking a dedicated Intake & Discharge Coordinator to oversee and coordinate all clinical intakes and discharges for adolescents entering or transitioning from our residential program. This role ensures a smooth, supportive, and professional experience for clients and their families from the first point of contact to the completion of care.

Intake Duties

  • Coordinate with the Admissions and Clinical teams to facilitate all admissions and discharges from start to finish.
  • Conduct intake assessments and ensure all required paperwork is completed.
  • Introduce clients to the milieu, assist with belongings, and manage all intake-related needs
  • Facilitate location tours, phone assessments, and other admissions tasks as needed.
  • Participate in onboarding training for new staff related to intake and discharge processes.
  • Maintain accurate chart audits and uphold the agency's philosophy and standards.
Discharge Duties
  • Coordinate all aspects of the discharge process between clinical and admissions teams.
  • Ensure all items and documentation are returned to clients and families.
  • Confirm all discharge paperwork is complete and compliant.
Professional Responsibilities
  • Maintain effective communication with clients, families, referral sources, and internal teams.
  • Attend staff meetings, treatment team meetings, trainings, and workshops.
  • Assist with outcomes research by gathering feedback and assessments.
  • Maintain professionalism in appearance, communication, and behavior at all times.
Schedule: Full-Time - Friday through Monday, 9:00 AM to 7:00 PM

Preferred Qualifications:
  • Bachelor's degree in a related field (or equivalent combination of education and experience).
Required/Preferred Experience:
  • Minimum 2 years in residential or facilities management/supervision (preferred).
  • Experience in residential or behavioral healthcare settings (preferred)
  • At least 1 year of experience working with youth and families.
