Coordinate and schedule care services for clients based on their individual needs and preferences - Work closely with the Care Manager to create and update care plans for clients - Communicate regularly with caregivers to assess their needs, desired schedules, and provide support and guidance - Maintain accurate records of client care, progress, and any changes in their condition - Collaborate with other healthcare professionals, such as nurses and therapists, to ensure coordinated and comprehensive care for clients - Assist with training, and supervising care staff - Participate in team meetings and provide updates on client care and progress - Handle any client or staff concerns or issues promptly and professionally - Stay up-to-date on industry trends and best practices in home care services.
