The Emergency/Transitional Team serves as on-site support staff where they will support Trueblood class members-individuals with behavioral health needs involved in the criminal justice system-who are eligible for emergency and transitional housing with the goal of obtaining permanent supportive housing.

The Emergency/Transitional Team will provide:
  • On-site support to residents, ensuring a safe and welcoming environment,
  • Intensive case management to TBCMs residing in emergency or transitioning housing.
  • Develop and implement individualized service plans focused on housing stability, behavioral health, and community reintegration.
  • Coordinate access to mental health, substance use, medical and social services.
  • Support clients in navigating legal obligations, including court appearances and compliance with diversion programs.
  • Collaborate with peer support specialists, health care providers, and employment coaches.
  • Maintain accurate documentation and participate in multidisciplinary team meetings.
The Clinician I is a member of a multi-disciplinary team which includes peers, a SUDP, Bachelors, and Master's level clinical staff. This team provides outreach and engagement services to individuals who have been involved with the forensic mental health system. This Clinician I works in close collaboration with other Trueblood Settlement Programs, to include Outpatient Competency Restoration, Forensic Navigators, and Trueblood Housing, to locate, engage and build rapport with eligible individuals in order to provide case management and housing support services. The team will not duplicate services offered through health and behavioral health benefits provided under other programs but will leverage services otherwise available to enhance the services available to enrolled individuals.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
  • Locate individuals identified on the HCA referral list.
  • Create or further develop recovery plans and on-going support to promote progress in care.
  • Provision crisis and hospital diversion services to individuals at high risk.
  • Complete suicide and homicide risk assessments to determine a level of response needed to maintain the safety of the client or others.
  • Assist non-Medicaid individuals to access Medicaid as appropriate.
  • Assist with obtaining SOAR services.
  • Provide evaluation and treatment for patients who have behavioral health concerns.
  • Assist and complete care plans and provide assessments and referrals for outpatient or inpatient treatment.
  • Provide consultation, training, and education to staff regarding behavioral health issues as assigned by supervisor.
  • Facilitate use of/access to crisis hospital diversion resources, including Stabilization and/or E&T admission.
  • Identify and include the client's support network in care coordination
  • Assist clients in identifying and accessing resources which will meet their needs.
  • Coordinate and collaborate with community resources on behalf of clients.
  • Maintain client records and all other required documentation in a timely and confidential manner, in accordance with agency policies and practices.
  • The above services will be provided by TB Housing staff only when the client is not receiving services from FPATH or FHARPS.
  • Requires travel to see clients in the community and travel between work sites.
  • Other duties as assigned.
QUALIFIED APPLICANTS WILL HAVE:

REQUIRED:
  • Bachelor's degree in psychology, counseling, social work or related field from accredited college or university.
  • Must apply for a Counselor Agency Affiliated - Registered credential from the WA State Department of Health within 30 days of hire.
  • Must have an unrestricted and valid driver's license, active car insurance and must be willing to use their personal vehicle in the course of work.
PREFERRED:
  • Two years of experience working with psychiatrically impaired populations.
  • Experience in the criminal justice system.
  • Experience working in housing programs.
  • Experience creating and conducting educational and/or skill building groups.
KNOWLEDGE, SKILLS & ABILITIES:
  • Ability to communicate and engage effectively with individuals of varied backgrounds, trauma experiences, cultures, education level and socio-economic status.

  • This position provides client services in community settings on a daily basis.

  • Daily travel between locations is required.
  • The ability to drive a vehicle, traverse distances up to several city blocks and on various terrains, navigate stairs, get in and out of vehicles frequently is required.
  • Daily use of a computer is required.
  • May be required to lift 10 - 20 pounds.
  • Blood-borne Pathogen Category III - No predictable exposure to blood-borne pathogens.
PAY RANGE (Hourly/ Non-Exempt): $25.89-$31.53

All pay is based on longevity at Frontier Behavioral Health; thus, the starting base pay for this position is $25.89 per hour.
