Provide case management, counseling, facilitate group and develop treatment plans and goals.
Prepare client admission to the facility by completing client screening, assessing potential medical problems, recording client vitals and complete all agency forms within admission packets.
Responsible for conducting educational and process groups for clients.
Medication management
Ensures proper progress notes and documentation within client files and the Electronic Medical Records system.
Follow individual service plan as created by mental health staff.
Follow treatment protocols and facilitate the motivation of clients to participate in further treatment.
Understand and have a working knowledge of all STARS, Mental Health and other social service agencies within Spokane County and to facilitate client movement through identified systems.
Prepare and complete referral and discharge forms.
Communicate all critical information to incoming staff as needed at shift change.
Perform Crisis Intervention as needed.
Maintain SUDPT license and qualifications.
Responsible for having a thorough understanding of the contracts between STARS and Spokane County Community Services Division.
Ensure the safety of all clients and staff by adhering to STARS policies and procedures.
Adhere to all HIPAA laws and regulations to ensure client confidentiality.
Attend all mandatory staff meetings.
Attend Supervisory Trainings monthly.
Performs other related duties as assigned by management.
Associate degree (AA) or equivalent from a two-year college or technical school, or six months to one-year related experience and/or training, or equivalent combination of education and experience.
Must obtain minimum education requirements per WAC246-811-030 to become a Substance Use Disorder Professional and maintain Washington state SUDP/T license and qualifications.
