About the Role:

*This is a remote telehealth position. Candidates must hold a valid New York State license.

Care2U is seeking two experienced NPs or PAs with active New York licenses to supportour home-based care platform by managing diagnostic results and coordinating patient follow-up remotely. This role is ideal for senior clinicians or retirees with 5+ years of experience who enjoy talking with patients and providing clinical care beyond the traditional office visit.

The position is primarily asynchronous, with concentrated responsibilities early in the day and flexibility in the afternoon and evening as long as the provider remains phone/chat available.

Schedule Requirements

• 3 consistent weekdays per week
• 3 weekend days per month
• On-call (phone only) from 8:30 AM - 9:00 PM
• Expect ~3-4 active hours in the morning
• The remainder of the day is lighter, with intermittent needs
• After morning queue of Results Management are addressed, APPs may engage in other activities as desired, as long as they are responsive to chat communications, stay reachable by phone and have ready access to the Electronic Medical Record.

Key Responsibilities

• Complete morning rounds from 8:30-9:00 AM for acute home-care patients
• Manage and follow up on diagnostic results
• Provide telephone support for critical results and write prescriptions and order referrals as needed.
• Document patient conversations in the Electronic Medical Record
• Escalate care as needed within the structure: Nurse → APP → Physician
• Maintain clear, professional communication and confidentiality

Compensation

• $250 flat rate per shift (8:30 AM - 9:00 PM), with the majority of call activity occurring in the first 3-4 hours
• Approx. $36,000 annually with the expected schedule
