Job Description:

As a Social Worker Case Manager, you will provide professional social work services to medical surgical, ICU, mother baby, labor and delivery patients, and offer ER coverage on weekends. Responsibilities include conducting mental health evaluations, planning discharges, and assisting with telepsych evaluations.

Responsibilities:

• Conduct EPIC system documentation and data entry.
• Perform drug and alcohol screenings using SBIRT.
• Conduct mental health evaluations using SAFE-T.
• Develop and manage behavior plans and capacity evaluations.
• Facilitate end-of-life conversations and discharge planning.
• Assist with difficult placements and provide telepsych evaluations.

Requirements:

• Minimum of 2 years acute care hospital experience, or 1 year acute plus 1 year in hospice or working with the elderly.
• Required certifications: MSW, LICSW, or LSWAIC (if no supervision by MHS is needed).
• Skills in EPIC, mental health and capacity evaluations, behavior plans, and telepsych evaluations.
• Experience with home health, hospice, SNFs, Medicaid/Medicare, and insurance authorizations.
